“Inadequate” NHS trust services

Services at Wye Valley NHS Trust Hereford Hospital have been found to be “inadequate” upon inspection by the Trust Development Authority in October 2014. The inspection resulted in the hospital being placed in special measures, stating that particular concern was raised to medical care and emergency services. In February of 2015 an improvement director was appointed by the Trust and a further announced inspection took place in September of 2015. Yet a further inspection, this time unannounced, was carried out at the end of September and focus groups were held to discuss the areas of improvement required.

It was found that improvements were necessary to ensure that the hospital delivered safe and responsive care to patients’ needs. Out of the eight core services at the hospital, five were deemed inadequate for safety. It was found that recruitment posed a significant threat with very high vacancy levels across both nursing and medical staff. In some areas, vacancy levels were as much as 40% and temporary staff were being taken on throughout. At the time of the inspection it was found that training of temporary staff was lacking.
The Care Quality Commission also found that mortality rates at the hospital were above the expected range of 100 with a value of 114. National standards with regard to patient admittance, referral or discharge within four hours in the emergency department were not being met. It was also found that there was a lack of knowledge among staff with regards to making safeguarding referrals and that incident reporting and management was ineffective.
